Our Evolution plans offer a variety of deductible, coinsurance, copays, and out-of-pocket options so you can customize a benefit solution to meet your needs. Our Evolution Value plan offers similar benefits to our Evolution Plan with higher out-of-pocket options to encourage in-network usage.
HDHP
Our Evolution High Deductible Health Plan (HDHP) offers a variety of deductible, coinsurance, and out-of-pocket options. These plans are meant to save premium dollars and promote health advocacy. The Evolution Value plan features higher out-of-pocket options to encourage in-network usage and can help drive down premium even further. Our HDHP plans can be paired with an HSA (Health Savings Account) to simplify account management.
